Insurance Information
We Are Pleased You Have Dental Insurance
Our office staff understands dental insurance and will be happy to assist you in obtaining maximum benefits specified in your contract. We will process your claims and accept assignment of benefits with the following agreement:
1. Your dental benefits program is a contract between you, your employer and the insurance carrier. We are not a party to that agreement.
2. Our fees are generally, but not always, covered in the full by the maximum allowance determined by your carrier.
3. Not all our dental services are a covered benefit in all contracts. Our office recommends treatment based on your individual needs, not what your insurance will allow.
4. You are responsible for all fees for service rendered to you.
5. The estimated co-pay must be made at the time of service. If the insurance pays less than anticipated, any amount not covered will be paid in full by you within 30 days.
6. If the insurance company pays more than anticipated, we will issue a refund to you, the patient, within 30 days. If the insurance company overpays in error, we will issue a refund to the insurance company directly.

7. You will supply us with accurate information regarding you,your employer, policy, and any personal information needed for submission.
8. You agree to follow the terms outlined in the FINANCIAL OPTIONS form listed in this web site. This form will be signed at your appointment.
Any of the business staff members will be glad to discuss your proposed dental treatment and answer any questions you might have as to the involvement of your dental benefits program in receiving care.
HMO PATIENTS
If a patient comes to us with a problem they expect to be covered by medical insurance (biopsies, tumors, TMJ, infections, pre-transplant, etc.), they must have a referral from their primary care physician. A referral from a dentist is not adequate for medical insurance coverage. Obtaining a medical referral is the patients responsibility. We cannot obtain the referral for you, and the referral cannot be obtained retroactively. If you do not have a referral, we will be happy to see you on a cash basis, but your medical insurance will not pay for your treatment.
